The primary care program offers a wide range of services to registered clients. The program services include: prevention, diagnosis, management and treatment. We provide a multidisciplinary approach to primary care and our clinical team consists of:

- Nurse Practitioners
- Registered Nurses
- Registered Practical Nurses
- Case Manager
- Registered Dietitian
Our clinical team works together to coordinate the best possible care for your health needs. We encourage all clients to take charge of their health and actively participate in their care.
Primary care clients can now cancel their appointment with their physician or nurse practitioner or send them a private document. All they need is their health card number and birth date.

In order to receive primary care services, residents of West Nipissing must complete a registration form, available in the “Become a Client” section of our Website. If you wish to become a client of the Centre, please fill out the registration form and return it in person to our reception desk, by fax to 705-753-5387, by email at info@cscno-wnchc.org, or by mail to the West Nipissing Community Health Centre, 68 Michaud St, P.O. Box 6308, Sturgeon Falls, ON, P2B 1B8. You can also complete an online registration form here.
WNCHC also offers a variety of community-based services and programs in health promotion and disease prevention, through education sessions, exercise and socialization groups. Participation in community activities is available to all residents of West Nipissing and not only registered clients.
